New Patients
If you have an appointment at Finnie Chiropractic Center, please arrive 15 minutes before your appointment to fill out forms or you can download the new patient forms.
Bring insurance cards and list of medications that you are currently
taking. Co-payments need to be paid at time of service and you are
responsible for co-payments or deductible that will not be paid by your
insurance policy. As a courtesy, we will file your insurance and
support you in the billing and obtaining of reimbursement from your
insurance company.
